APPLICATION PORCEDURES FOR

MANUFACTURING, CONSTRUCTION, SERVICE & AGRICULTURAL SECTORS

STAGE ONE - APPLICATION PREPARATIONS

  1. To register with Jabatan Tenaga Kerja (JCS, 63A, 60K)
  2. Preparation of the following documents for submission to KDN :-

    a) Completed Application Form

    b) Form 9, 24, 49 (Sdn Bhd) / Borang B & D / E (Sole Proprietor) - Certified by SSM attach with Original Receipt

    c) Employer Identity Card (I/C)

    d) Original & Copy of 3 months KWSP Statement

    e) Original & Copy of Latest Audit Account

    f) Original & Copy of 3 months Bank Statement

    g) Original & Copy of JCS

    h) Original & Copy of FWCS

    i) Original & Copy of Check Out Memo or cancellation Report From Immigration

CONSTRUCTION : -

  • Original & Copy CIDB CERTIFICATE
  • Original Letter of Award for ongoing projects
  • Original CIDB Levy or CIDB L2/96
  • Complete form for "foreign workers distribution affirmation"

MANUFACTURING : -

  • Original & Copy of MITI License (for company with more than 2.5 million paid up capital or with more than 75 workers)
  • Original & Copy of 3 months K2 Form
  • Profile, photo of premises, machineries and products

AGRICULTURE :-

  • Copy of Land Title for land Owner
  • Original & Copy of Tenancy Agreement - if any
  • Original & Copy of 'Letter Of Consent by Jabatan Pertanian

SERVICING :-

  • Original & Copy of Merchant License
  • Original & Copy of Stamped Letter Award (Cleaning Service)
  • Original & Copy of Memorandum and Articles (M&A) (Cleaning Service)
  • Original & Copy of Stamped Tenancy Agreement for a least 3 years (Cleaning Service)
  • Photo of merchant's premises

STAGE TWO - INTERVIEW AT PUTRAJAYA

  1. Upon receipt of the completed documents we will complete the application booklet, provide transportation for employers to interview at KDN Putrajaya.
  2. Purchase bank draft payable to 'KETUA PENGARAH IMIGRESEN MALAYSIA' for each worker applied
  3. The approval expires within twelve (12) months & extendable for another 6 months.

STAGE THREE - RECRUITMENT PROCESS

Employers have the option to bring in either all or in batches the workers who are being approved during the KDN Approval validated period. The procedures are as below :-

  1. Demand letter, service contract & recruitment contract to be attested at the respective embassy's country of origin, where biodata of workers will be provided.
  2. Submission of biodata (copy of passport, photo & medical report) of workers with approval, insurance guarantee, FWHS, Form IMM 12, ROC to the state immigration for the processing of caling visa.
  3. Sending the calling visa to the worker's country of origin, where our recruiters will have the visa endorsed on the passports, whereby the workers are ready to depart to Malaysia.

STAGE FOUR - UPON DISEMBARKATION

  1. FWCS (Insurance) will commence on the next working day after arrival.
  2. Workers are required by Fomema Sdn Bhd to undergo a medical checkup within a month upon arrival.
  3. Upon confirmation of workers who are medically fit to work, the passports will be submitted to immigration for endorsement of permit sticker.
